You have two choices there: the FormFields and ActiveX controls. If you need dynamic, go to the Developer tab, Controls group, and look in "Legacy controls". If they can be static, I'd insert an appropriate "symbol" (Insert/Symbols) Could you explain what's to happen with these documents, once they've been generated? Will users be working with them (checking or unchecking the checkboxes), or will they be static?
